This form will be received by Soquel's social-emotional team, and seen by a member of our social-emotional team. Students will be seen within one week.

         If possible, let the student know you are making the referral. We understand this is not always possible. Do not let this be a barrier to sharing your concerns with us.

       *Please do not use this referral form for:

1.Emergency situations - call 911 and contact a school administrator ASAP,
2. Suspicion of child abuse - YOU (school employees) are a mandated reporter and are responsible for reporting directly to Child Protective Services at (831) 454-4222
3. If the student is actively experiencing a mental health crisis - immediately alert a school administrator, school counselor, or school social worker.
