Informed Consent
You are invited to participate in a study by Tyler Robinson from The Willow School(senior). Your participation in this survey is voluntary. Please read the following information carefully before deciding whether or not to participate.
Purpose of the Study: This study aims to understand effects of athletic injuries in college and high school students and how it affects mental health to understand if there is a correlation between the type of injury and how it affects someone's mental health. Your responses will help determine if there is a correlation.
Procedures: You will complete an online survey that will take approximately 15 minutes. It includes questions about age, gender, type of sport played, type of injury, amount of time absence, and type of sport.
Confidentiality: Your responses are confidential and no identifiable information will be collected. Data will be securely stored and reported in aggregate form, ensuring individual anonymity.
Risks and Benefits: There are minimal risks, and you may skip any uncomfortable questions. While your participation will not directly benefit you, it will contribute to valuable societal research.
