1. 截止提名日期為2023年6月30日(星期五)下午五時。逾期遞交的提名將不獲受理。
The nomination deadline is 30 June 2023 (Friday) at 5:00pm. Late submissions will not be processed.

2. 以傳真提交的提名表格將不獲受理。
Submission of nomination form by fax will not be processed.

3. 以郵寄／網上方式遞交的有效提名，本會將於2023年7月中或以前以郵寄方式向獲提名的老師及相關校長發出認收信。如於2023年7月14日仍未收到有關通知，請與本會聯絡。
For those valid nominations which are submitted by post or online, acknowledgement letters will be sent to the nominated teachers and relevant school heads by post by mid-July 2023. Please contact us if the nominees have not received the letter by 14 July 2023.

4. 獲表揚教師的姓名將印刷在表揚狀上，請依照香港身份證上的姓名用正楷填寫。
The name of the commended teacher will be printed on the certificate. Please fill in the name as indicated on the HKID card.

5. 本提名表格必須填上正確的學校編號及檢定／准用教員編號，方為有效。
This nomination is only valid with correct School Number and Registered / Permitted Teacher Reference Number.

6. 收集的資料只用於與表揚教師計劃有關的活動。
Information collected will only be used for the Teacher Commendation Scheme.

7. 如有任何爭議，本會將保留最終決定權
In the case of disputes, Committee on Respect Our Teachers Campaign reserves the right of final decision.
