HCSD Bus Emergency Form for Returning Students
Complete one form per student.

HCSD requires each student to have a completed Bus Emergency Form before riding a bus. In addition, this form must be complete before your student can return to school from distance learning.

All information entered here must match the information given at the time of registration. The student may only be picked up and dropped off at the address listed on the school registration unless approved for an alternate pick up/drop off location. Forms submitted after 12:00 pm, will not be processed until following day.
