Upon receipt of this form, you will receive a confirmation email stating a seat has been reserved for your student in the selected class with Amy Craven Tutorials and you will receive an invoice for the non-refundable registration fee ($15) and tuition ($450). Payment is due upon receipt, unless you have requested the Payment Plan on this form.

The Payment Plan consists of three (3) monthly installments of $150 payable by check. The first payment is due upon receipt of the invoice ($150 + $15 non-refundable registration fee). The next two payments of $150 each are due monthly thereafter, with the entire amount due before the first class on August 20, 2020.

In the event a student should drop the tutorial prior to the second class meeting, which is August 27, 2020, 50% of tuition funds minus all transaction fees will be refunded. After this date, no reimbursement will be made as it is unlikely the seat can be filled.

In the event the class is canceled due to <4 students completing their enrollment or other unforeseen circumstances, a full refund will be issued.
