The following forms can be completed online, with the exception of the Release of Information form. If you would prefer to complete a paper copy please let me know.
- Complete Consent for Services form (this form).
- Complete Intake Assessment in full.
- Complete Strengths and Difficulties Questionnaire.
- Complete Release of Information (ROI), if applicable, so I can collaborate with outside professionals (**for example, If your student(s) receives outside counseling services). This form will be sent home or can be completed in person at the school.
