What are your experiences during the Covid-19 Lockdown in education, health and social care?
Who is the survey for?
It’s for all families in Walsall with children and young people who have special educational needs and disabilities (SEND).

Who’s asking?
The survey is being carried out by FACE Walsall which is the local parent carer forum.

Why do you want to know?
We need your help to correctly inform the Local Authority, Health and Social Care of what our lives have been like during lockdown. This information will help everyone to understand what is needed in Walsall.

What are you going to do with my answers?
We will use them to write a report that we will give to the people in Walsall who are responsible for providing services for our children in education, health and social care. Your answers will be kept anonymous.

Do I have to answer the questions?
No; if you do not want to take part then that is absolutely fine. You can also choose to answer some or all of the questions. If you do answer them we will only use them in the way we have described.

When do I have to answer the questions by?
The closing date for the survey is 28th February 2021.
