I agree and understand that swimming is a hazardous activity. I recognize that there are risks inherent in the sport of swimming, including but not limited to paralyzing injuries and death.

I hereby agree to indemnify and hold harmless the Greenville Area Summer Swim League, its coaches, officers, directors, agents, volunteers and employees, and its swim teams and their respective coaches, directors, officers, agents, volunteers and employees against any liability resulting from an injury that may occur to the participants while participating in the Greenville Area Summer Swim League program.

I authorize any representative of the Greenville Area Summer Swim League to have the participant treated in any medical emergency during their participation in the program. Further, the participant and/or guardian agree to pay all costs associated with medical care and transportation for the participant.

I will notify my child's coaches of any medical/health problems of which they should be aware.
