Every league member must read and accept this release and waiver before participating in any 2025 Golf Activities at Centennial Park Golf Centre Inc. (CPGC).

By agreeing below, I confirm that I have read, understand, and agree that there are inherent risks associated with each of the following activities (referred to as “Golf Activities” in this waiver):

-the game of golf and practicing on the driving range and practice greens

-the use of golf equipment, whether rented or purchased from CPGC or whether my own, and the use of power carts and pull carts

-the use of CPGC facilities, whether indoor or outdoor

-participation in sports, fitness, or instructional activities provided or organized by CPGC

-participation in any other activities, services, programs, or events provided or organized by CPGC

-use of the food and beverage services provided or organized by CPGC

I am aware that the risks include, but are not limited to, the potential for serious personal injury, death, or property damage. I understand that the risks may be relative to my own state of fitness and health (including physical, mental, and emotional), and to the awareness, care, and skill with which I conduct myself. I must be aware of other golfers, golf balls, golf clubs/equipment, power carts, cars, maintenance equipment, uneven grounds, animals, and wildlife.

I agree that golf and etiquette have traditionally gone hand in hand.

I agree to observe a code of practice that places safety (my own & others), courtesy, respect, and fair play at the heart of my golf activities at CPGC.

I agree to release and discharge Centennial Park Golf Centre Inc., its owners, directors, managers, employees, coaches, instructors, and volunteers from all liability for all personal injury, death, property damage, or other loss resulting from my participation in Golf Activities in the calendar year of 2025. This release and discharge applies even if such injury, death, property damage, or other loss results from, is contributed to by, the negligence of Centennial Park Golf Centre Inc., its owners, directors, managers, employees, coaches, instructors, or volunteers.

In consideration for being allowed to participate in Golf Activities at CPGC, I agree to the terms of this Waiver and Agreement.
