For the safety of our students, we are initiating the following check-out policy for the school year.

Your child will be released only to the people named on this form, which is kept on file in the office.
All persons checking students out should be 18 years of age or older.
Please make sure the name(s) listed  below are the legal name(s) listed on that person’s ID, as they will be asked to present their ID when checking a student out.

We also collect annually permission status for over-the-counter medications and photo sharing.
