Carlisle HIPPY 2025-2026 Pre-Registration Form

By filling out the form below, this will let us know that you want a spot for the 2025-2026 school year for your child. Once we have this information, we will then contact you once we come back after summer break.
Then, you will fill out our required paperwork, and turn in copies of the following REQUIRED documents:

- Child's Birth Certificate (hospital birth card does not work, it must be a state issued certificate)
- Child's Social Security Card
- Child's most recent Well Child Check
- Child's shot record

We must have these documents, as they are required by the State. Carlisle HIPPY is offered FREE to the families within the Carlisle School District area. Your Child can be 2 by August 1st or Your child must be at least 3 or 4 by December 31st.
