Please select the date and time you would like to attend class.  Program will be held December 11 10:00-12:00 and 1:00-3:00 and includes a 1 hr. program followed by 1 hour of play in the galleries.  Space will be limited to ensure social distancing.  Please mail in or drop off payment within 3 days from receipt of confirmation.

Any guest exhibiting cold or flu like symptoms such as fever, vomiting, diarrhea or excessive coughing or runny nose will not be permitted in the Center until they are symptom free for 24 hours.  The Center reserves the right to request visitors to leave the Center or one of our programs if you or your child is sick.

In consideration of accepting this entry, I, the undersigned intending to be legally bound for myself and/or my child, and our heirs, executors and administrators, waive and release any and all rights and claims, injuries and damages I and/or my child may have against the Children’s Discovery Center, and all volunteers, sponsors, and the officers, directors, agents and employees of any of them.  I understand that any public place where people gather presents an inherent risk of exposure to COVID-19. I will abide by the Children’s Discovery Center’s policies and procedures to mitigate that risk for myself and my family. I additionally consent to the use of my and/or my child’s name and/or picture in broadcasts, telecasts, etc. arising out of participation in Discovery Center events without any payment to me and/or my child.

Please mail or drop off payment within 3 days of receiving a confirmation.  Checks can be mailed to: Children's Discovery Center, 111 Ohe St., Honolulu, HI 96813.   Workshop is subject to cancellation if the program does not meet enrollment requirements and payments will be refunded.  There will be no refunds for cancellations once payment has been received.  A $30 bank fee will be assessed to any returned check.
