Thank you for your interest in our mentoring scheme. Please complete the form below with as much care as possible. All contact details will be held in confidence and used only for communication between you and the scheme co-ordinators.

Selection for the scheme will be based on a three stage process:

1. Submit application by the deadline.
2. Attend a group interview during October.
3. Attend the induction session in November.

If you are unable to complete any of these stages, your application will not be taken any further.
The following criteria will be used to assess your suitability for the scheme:

1. Career need - 10%
2. Motivation and commitment to the scheme - 40%
3. Quality of your written application - 10%
4. Availability of suitable mentors for your interests - 40%

I agree to attend the induction session and final celebration event; I accept responsibility to keep in regular contact wtih my mentor; I agree that meetings should take place in a public place (preferably the mentor's place of work or the university); I agree to keep the scheme co-ordinators informed if any difficulties occur; I agree to contact my mentor and the scheme co-ordinators if (for whatever reason), I decide to withdraw from the scheme.
