By submitting this application, you acknowledge that you are the parent or student applying. Once your locker combination is issued, you may not allow other students to use your locker. Improper use of your locker could result in loss of locker privileges and/or disciplinary action.

Lockers cannot be purchased unless all school fees have been paid. Once you have completed the form, you will receive a copy of your response via email. You will get your locker number and combination on Rebel Night if paid in full. Top lockers will be given to those on a "first-come, first-serve" basis. The form records the date and time that an application is submitted so we will use the timestamp to give priority.

Tardies to class may result in suspension of locker privileges. Excessive tardies may result in loss of locker privileges for remainder of school year.

Locker Rental Payment will not be refunded if locker privileges are revoked.

Thorsby High School will not be held responsible for any theft or damage to any student belongings inside the lockers. We provide the lock; it is the student's responsibility to ensure it closes.

Lockers may be searched/checked at any time by administration.

Lockers rentals are not refundable once purchased.

Lockers are NOT to be shared. The person who rents the locker is responsible for everything inside the locker.
