VITA Tax Appointment Scheduling Tool: Site Admin Log-In Request Form
To opt-in to our VITA Tax Appointment Scheduling system, fill out registration form to request site-admin access. As a site admin, you agree to:

1) Display your sites and appointments available in free tax prep finder at
2) Promote the use of self-appointment scheduling among your clients by providing a link/hyperlink to on your website or materials so clients can make an appointment on their own.
3) Provide UWCA/UWBA with your appointment dates and times for master upload for each of your sites. Upon registration UWCA will send you a spreadsheet template to complete that can be returned to us for upload or uploaded on your own. (Refer to site admin user guide for more information).
4) Update any changes to appointments available and review reports of clients to ensure follow-up and continuity of service.

To opt-in and be eligible for incentive, you must be in the San Mateo, Santa Clara, or San Francisco County area. There is no opt-in registration deadline. Incentive grant recipients will be determined by 1) the size of the organization (based on 2016 returns) and 2) number of appointments made using the tax appointment scheduling system.
