Michigan Technological University students, faculty, and staff who have traveled in the past 14 days are asked to fill out the form below if their travel could have exposed them to the coronavirus disease 2019 (COVID-19) or if you think you have been exposed in other ways. The information collected will help University and state health officials prevent the spread of COVID-19 and maintain the health and safety of the University and our community.
