1. Signed Vendor Affirmation and Liability Release form
2. Signed Non Food Vendor Rules and Instructions
3. Payment Check made out to "Music Festival of Rosendale, INC."

Space Options: All spots are 10' by 10' and are on the street which must be broken down overnight Saturday.
Non resident booth ($125 before June 1st, $145 after)
Main Street Rosendale Business Booth ($100) Must be applied for by June 1st to ensure placement in front of your business. Please note that if you want more then one spot, they are at the non resident price after the first.
Main Street Rosendale Resident Booth ($75) Must be applied for by June 1st to ensure preferred placement. Please note that if you want more then one spot then they will be the non resident booth price after the first.

Electricity Option: 110 volt outlets available at $60 each. Please indicate how many outlets and the amps needed below. If not needed put n/a.
I understand that in addition to this form, there are various things that I need to mail in with my payment as indicated at the top. Furthermore I know that my spot is not set until all needed items are turned in.
