Přihlašuji se tímto ke členství v DiscDog Klubu České republiky (dále jen "Klub") a zavazuji se dodržovat řády a směrnice klubem vydané. Souhlasím, aby uvedená data sloužila jako podklady pro činnost Klubu a s jejich zveřejněním v souvislosti s činností Klubu, včetně zveřejnění na internetových stránkách Klubu. Dále souhlasím s jejich předáním zpracovateli, pořadatelům vnitrostátních i mezinárodních soutěží a souhlasím se zveřejňováním svých osobních údajů a sportovního výkonu nebo funkce (rozhodčí, předseda a pod.). *
This is my application for Czech DiscDog Club membership and I pledge to follow all the rules and regulations set by the Club. I agree that my personal data (first and second name, date of birth, phone number, address etc.) will be added into a database. I agree that these data can be used for the Club´s operation and that they can be published if it is related to the Club´s operation, including releasing these data on the Club´s official websites. I also agree that these data will be passed on a person responsible for the data´s processing and on event organisers. I agree with publishing of my personal data, my competition ahcievements and/or my post (judge, chairperson etc.).
