All eligible applicants must have:
- a valid medical recommendation
- a valid identification (Driver's License, state-issued ID, passport, etc.)
- a residence address for delivery; deliveries will not be made to office buildings or parking lots

Before acceptance into the program, applicants will be required to provide copies of their medical recommendation and ID.
