WCVM Equine Education Day 2025 PAYMENT
Equine Education Day is February 1st, 2025 
Registration Deadline is January 18th, 2025

IMPORTANT INFORMATION

This document must be filled out by the person/club submitting the payment
INDIVIDUALS - if you are paying for a singular participant you MUST fill out both the registration form and payment form for registration to be complete
CLUBS -  If you are paying for an entire club, each participant must fill out the registration form and then one club member that is paying MUST fill out the payment form:

If submitting an e-transfer as payment - please Include either the individual members NAME or the CLUB NAME in the message.

**Cheques can be made out and paid day of - at the door

No password is required for the e-transfer.
**PLEASE include the name/names of the registrants included in the e-transfer** IF NO NAMES ARE INCLUDED YOUR REGISTRATION WILL BE CONSIDERED INCOMPLETE
Please indicate all registration payment totals that will be expected via the payment.
If a club is paying for multiple members please indicate the following information: 
- Club name 
- Who is making the payment (Eg "Club President (Name)).
- All member names being paid for
