This checklist includes all documents required to apply for a CRBA and U.S. Passport for your child. Please print out this checklist, sign it, and bring it to your appointment with your signature and date. (Each child applying for a CRBA must schedule a separate CRBA appointment by following the instructions. Multiple children CANNOT be accommodated on one CRBA appointment - each child needs his/her own appointment.

How does eCRBA work?
You will start by opening a MyTravelGov account. Once you have an account, you can create an eCRBA application, which will guide you step by step through the process of applying online, including uploading all necessary supporting documents to ensure that the application is complete. Once the application is complete, you need to pay the eCRBA fee online. Once the payment is complete, you will be prompted to schedule an in-person interview at U.S. Consulate General in Frankfurt. You must bring the child and the originals of all supporting documents to the in-person interview, where a consular officer will interview you to verify the information provided, review the original documents, and decide whether to approve the application.

All required documents require that you submit both the original documents and 2 photocopies of each original.

The documents should be placed in the same order as the checklist with the original on top of the copies. If you do not bring all necessary below documents or copies as indicated, you will be asked to make a new appointment. If a document is not in English or German a certified English translation is required.

Consular Report of Birth Abroad:

❑ Online eCRBA application form and pay online. HERE You should have scanned all below documents into your online application, if you have not done so, please email us all below necessary documents at FrankfurtPassports@state.gov with subject line "Additional documents CRBA for [Type Child’s Name]. Please bring your confirmation sheet, confirming that you have applied and paid the CRBA online.

❑ If you do not qualify to use the eCRBA, please bring completed form DS-2029. Your DS-2029 form must be completed electronically; we will not accept handwritten applications. Fill in the form as completely as possible, including all dates parent(s) was physically present in the United States during their lifetime. The application must be brought to your appointment. Do not sign the application. Print all applications and documents single sided. They can be printed on A4 or U.S. letter paper. Please email us all below necessary documents with the paper CRBAat least 2 days before your appointment at FrankfurtPassports@state.gov with subject line "Paper CRBA and documents for [Type Child’s Name]

❑ Child's German birth certificate ( Must be the "Beglaubigte Abschrift aus dem Geburtenregister" or "Auszug aus dem Geburtenregister"). Sample

This is the German birth certificate issued by local authorities only on request. We will not accept any other birth certificate. Note: Children born before January 1, 2009, may have been issued an "Abstammungsurkunde." which are also acceptable.

❑ Original evidence of the parent(s) U.S. citizenship: Valid U.S. Passport. If passport is not valid, we will need one of the following U.S. birth certificate; Consular Report of Birth Abroad; Certificate of Naturalization; or Certificate of Citizenship.

❑ Original valid Passport for the non-U.S. citizen parent. Examples include: Passport or a Passport ID card.

❑ If parents of child are married: Marriage certificate

❑ If either parent has been previously married: Need divorce decree, annulment decree, or death certificate for all previous marriages.

❑ Evidence of Name Change if either the mother’s or father’s current legal name does not appear the same on all presented documents, provide official evidence of a name change. Accepted evidence includes marriage licenses, divorce decrees, and official name change documents issued by the U.S. or a foreign government.

❑ For unmarried U.S. citizen fathers IF the father is not listed on the birth certificate, we also require a legal statement recognizing paternity ("Vaterschaftsanerkennung").

❑ If the U.S. citizen father will not be present on the day of the CRBA appointment, please have the father fill out and sign in front of a Notary public the DS-5507, Affidavit of Parentage and Physical Presence as well as the DS 3053, Statement of Consent form. The original notarized MUST be brought to the CRBA appointment.

❑ Evidence of physical presence in the United States: The U.S. citizen parent must provide proof that,

before the child’s birth, the parent spent the requisite amount of time in the United States needed to

transmit citizenship to their child born abroad. Documents that help demonstrate physical presence include but are not limited to: High School report cards/transcripts, university/college transcripts; Military Discharge DD-214; employment records; Social Security earnings summary. Please bring all documents you can find.

Child's U.S. Passport:

❑ Completed DS-11 form for the child's first passport at the same time. Complete the DS-11 form using the online form. We will not accept any handwritten application. The application must have a 2D barcode on the upper left-hand side of the first page. Because your child does not have a Social Security Number, you will need to enter "000-00-0000" in the field requesting the applicant's SSN. The application must be printed and brought to your appointment. Do not sign the application. Print all applications and documents single sided. They can be printed on A4 or U.S. letter paper.

❑ Social Security Statement: Signed and dated SSN statement. Please print out and include this affidavit with your child's passport application. Social Security Statement.

Once your child has the CRBA and Passport, you will need click this link: Frankfurt FBU Navigator to ready what documents and forms are needed to apply for a SSN.

❑ 2 x 2 inch photo of child, 2in x 2in (5cm x 5cm), set against a white background, taken within the last
six months. Eyeglasses are not permitted in photo. Photo Instructions When photographing babies, no other person, hand, or clothing should be visible on the photo.

❑ A self-addressed pre-paid DHL Express envelope that is a minimum C4 (DIN A4) in size. The envelope must not be padded. You must use the DHL Express Easy postage up to 500g and choose "Expressversand". The DHL label is ONLY available online.

You cannot purchase the DHL label or envelope at the Consulate.
You can only purchase the DHL Express label online and not at the German Post Office.
When purchasing the label, it will ask you for the "Absender" phone number, please provide your phone number and not the Consulates. Address: U.S. Consulate General, Attn: PPT Section, Giessener Str. 30, Frankfurt, Germany.
If applying for a book and card, you will need to provide us with two envelopes.
Processing time for the passport book is 7-8 weeks. The card will be mailed 3-4 weeks after the passport book has been delivered to you. The DHL Express shipping label you can only purchase DHL  Express Easy shipping label online. (choose “Expressversand”) attached to a non-padded A5 or C5 size envelope using your own email address for the “Absender”.

❑ The non-refundable CRBA and Passport fees must be paid on the day of your appointment. If you did the online eCRBA, you do not need to pay the CRBA at the consulate. The current fee for a CRBA is $100 and a minor passport is $135. The passport fee for a child over 16 is $165. We accept cash (euros and U.S. dollars) and Master and Visa credit card. You cannot use a credit card which requires authentication on a cellphone, since you are not allowed to bring any electronics into the building. We do NOT accept EC cards or personal checks.

❑ On the day of the CRBA appointment, both parents and child must be present. You must bring all original documents listed above and 2 photocopies of all documents. We do not have a copy or photo machine at the consulate.

❑ If one parent/guardian cannot go with the child to apply for the passport, they can give permission by completing Form DS-3053 “Statement of Consent." You must submit the completed form with the child's passport application. The parent that cannot go with the child must:

Sign and date Form DS-3053 in the presence of a certified notary public. While it is highly recommended that notaries fill out the corresponding on Form DS-3053, German notaries routinely use their own notary cover sheet. Please check to make sure that all details from the DS-3053 are reflected in the German notary's cover sheet, including the photo ID number.
Must submit a photocopy of the front and back side of the ID that you present to the notary public with Form DS-3053.

❑ If you are affiliated with DoD or fall under SOFA, please visit your military passport agent on base for a CRBA/Passport appointment.

❑ Due to very high demand, we are unable to change the date or time slot of your appointment.  Therefore, please make sure you come prepared. 

❑ IMPORTANT: The DS-2029 application is valid for 90 days from the day we received your online payment. You must complete the interview at the consulate before the 90-day deadline, or else your application will be considered abandoned, and you will be required re-apply and pay your fee again. 

❑ The processing time for the CRBA and Passport will be 7-8 weeks from the day of your appointment.
