State of Maryland Off-Site Permit Event Report
This is the form used to submit events covered under the Off-Site Permit issued by the Office of the Comptroller. The reports submitted via this form are sent to the Comptroller on the 20th of the month prior to the events.

If you file using this form, you do not need to file a Form 386 Intent to Participate paper form, though you must keep a copy of the form signed by the owner of the property granting you permission to participate.

Two changes based on requests from the Office of the Comptroller:
1) This form is now collecting responses from wineries, breweries and distilleries.
2) Local liquor boards now have access to the results of this form (to confirm your activities are permitted)

If you are submitting event after the deadline (i.e., after the 20th of the month prior), please select "Amended Report" and provide an explanation.
