Donation to ISD #192 Request Form
PROCESS to DONATE to ISD#192

All donations to the Farmington School District (ie coaches, equipment, bussing, etc) are made directly to ISD #192 using this request form.

All invoices must be paid by the district because as a non-profit organization, boosters should not own equipment nor do we want to be responsible for the repairs on any equipment that is purchased for our athletes to use. 

Once the request has been made, a check to the school to pay the invoice should be directed to Sue Hurtgen.

Bussing upgrade requests do not need to specify an amount.  Use TBD.  Sue Hurtgen will invoice the Boosters once the school has been invoiced for the bussing charge.
