2026 Field of Heroes Patronage Request Form

We are excited to be celebrating the 18th Anniversary of the Field of Heroes and would like to invite you to join our efforts by supporting our event as a Patron. The Field of Heroes is a Memorial Day tradition in central Ohio, providing an opportunity for our community to honor their personal heroes.

The 2026 event will be held from Friday, May 22 - Monday, May 25 at the Westerville Sports Complex, 325 Cleveland Ave. Westerville.

Upon the receipt of this form, you will be provided an invoice with payment instructions.

The deadline to submit this form and be included in print advertising (according to your patron level) is Friday, April 24, 2026.
