Please note: If you do not wish to upgrade your existing unit, the PR-5 REMAINS FORD APPROVED and will continue to work for intended purposes.

THE PROCESS: In order to receive a PR-5 Upgrade, please complete this form with all the required information. Pro Spot will process upgrades in the order they are received. When your upgrade is ready to be processed, Pro Spot will contact the person listed on this form and inform them on how to proceed.
