It is MANDATORY for all students to complete a field trip release form before participating in any field trip off campus.  This form only needs to be completed one time during your tenure as a KCAI student.

The undersigned hereby releases and forever discharges the Kansas City Art Institute, its trustees, officers, agents, and employees from any and all responsibility, liability, losses, claims, or damages arising out of or in connection with any field trips scheduled by KCAI or any of its departments.

Participant further agrees and understands that during any/all field trips he/she will be under the direction of the faculty member.  Participant agrees to comply with all reasonable directions and instructions by the faculty member during the course thereof.

Should participant require emergency medical treatment as a result of accident or illness arising during the field trip, I consent to such treatment.  I agree to be financially responsible for any medical bills incurred as a result of such emergency medical treatment.
