Internship Questionnaire
This form must be filled out in order to be considered for an internship that receives school credit. For those students seeking non-credit internships: filling out this form is important, as it will provide you access to our internship database.

All requests to take a for-credit internship for next semester are required to be completed, and approved, by the week of January 14th, 2019 (FOR SPRING 2019 CREDIT). Please consult with Suerynn Lee in the main Art Office for required paperwork for ART 39598: EDM Internship-as-Independent Study. The approval process takes time and requests turned in too close to the deadline run the risk of being rejected. You must have secured your internship and be enrolled in ART 39598 *PRIOR TO THE UPCOMING SEMESTER'S REGISTRATION DEADLINE* to take an internship for credit.

By filling out this form, you will be consulted when internship opportunities arise. You should make sure to renew your availability each semester.

This questionnaire is for EDM to help you find an internship, but we do not guarantee an internship to every student that wants one. Students are highly encouraged to seek out internship opportunities independently. (Check the CCNY Career Center, NAC 1/116, and our list of internship resources including our EDM Facebook page.)
