Team Registration Requirements:
1. Completion of this Team Registration Form
2. Waivers of Liability from Each Player (Follow this link to download the waiver and share it with your teammates: To continue the registration, the registrant will need to attach all completed waivers to this registration form, in Section 2, in order to submit the application.
3. Payment of Team Registration Fee ($400/Team - upon submission of this registration form, a link will appear to pay team registration fee)
4. Player Eligibility: All Players Must be Current USPA Members with a USPA Arena Handicap
5. Team Eligibility: Total Handicap of Team Must = 3-6 Goals (Arena Handicap)

Team captains are encouraged to register all potential teammates in this initial process. Substitutions will only be permitted once the Host Tournament Committee has confirmed that the substitute is a current USPA member with a qualifying arena handicap and he or she completes a waiver of liability. Last minute substitutions risk forfeiture or disqualification if eligibility requirements cannot be met. Entry fees are non-refundable after Wednesday, October 10th.

LOCATION: All tournament rounds will be played at the Westchester Polo Club Arena & Training Center in Portsmouth, RI.

ENTRY FEE includes:
Tournament matches, lunch Sat/Sun and Saturday evening player's dinner following the conclusion of the day's competition.


FRI 10/12:
TBD Arena Rules Review; Opening Round of Competition; Evening 'no-host' welcome dinner off-site;

SAT 10/13:
TBD Rounds of Competition
6PM Player's Dinner (on-site; 3 dinners per team included in registration fee, RSVP required for additional guests)

SUN 10/14:
TBD Consolation Round, Bronze Round, Championship, Trophy Presentation

*A complete registration requires meeting all of the above. Registration deadline is 5pm, Friday, Oct. 5. The tournament must have an even number of teams. The first 4 teams to meet all registration requirements will be admitted, the 5th team will be wait listed until a 6th team enters, and so on. Once registrations are received, the Tournament Committee will review and send notification by email to the Team Captain that registration was received and the team is confirmed/wait listed.
