This form should be filled out ONLY by individuals who identify as multiply marginalized and underrepresented scholars (MMU) on behalf of themselves. In general, MMU scholars identify as being one or more of the following: racial and/or ethnic minority, LGBTQ+ identity, disabled and/or neuro-diverse identity. However, this list is not exhaustive and is not meant to cause further marginalization.
