Below you will find information regarding parental consent for your child to be enrolled in the Shield Illinois program, which includes both covidSHIELD and binaxNOW testing.  Student participation in this program will limit potential quarantine situations and allow students to safely remain in an in-person learning environment.

Student participation in this program is NOT required.  However, parents are highly encouraged to enroll their child to help protect our school and community against the spread of COVID-19.  

If you would like to enroll your child in this program, please fill out the included information in this consent form.  

Please note that approximately 20-30% of enrolled participants will be selected randomly each week for testing.
