Day Middle School Absence/Tardy/Dismissal Reporting Form 2025-2026   
Day MS - Year 2025-2026 -  Please use this form to report your child's absence from school. Absences include full-day absences, late arrivals, and early dismissals. Please note that attendance in school is vital for building a sense of belonging and consistent academic performance.  To help your child achieve all they can, please limit absences, tardy arrivals and early departures to illness, urgent medical appointments or other unavoidable appointments.

***NOTE: Families will continue to receive district notifications about student absences and tardies by text, email, or phone call. Please note that the information you share with us through this Day School Google Form is for our school office records only. It is not connected to the district’s automated absence reporting system.Thank you for helping us keep attendance records accurate and up to date!

This form is for parental/guardian use only.  The request MUST be a Parent/Guardian email address that is currently in Aspen connected to your student's portal. Any student who misrepresents themselves as their guardian by filling out this form are subject to suspension for forgery.

Documentation:
Please provide a medical provider note or any documentation relating to your child's attendance.   You can email this document to wongs@newton.k12.ma.us.  Your student may also bring a note to the Main Office upon arrival.

Please be aware: Vacations/travel/family trips are NOT considered excused absences.
