I/We give permission for Mount St. Mary Academy to use my/our daughter's name as well as photographs, video footage and recordings that may be taken of her at school-related events and activities, both on and off campus, in communications and marketing pieces produced by the school and its approved partners. Such pieces may include, but are not limited to, promotional materials, press releases, newsletters, the school website and social media. I/We further acknowledge that I/we will not be compensated for these uses, and that MSM exclusively owns all rights to the images, videos and recordings it takes, and to any derivative works created from them. I/We waive the right to inspect or approve of the uses of any printed or electronic copy

We have read and understand the Mount St. Mary Academy Student and Parent Handbook.  As a student, I agree to abide by all policies and procedures outlined in this handbook.  As a parent/guardian, I have discussed these guidelines with my daughter and agree to abide by the policies and procedures established in the handbook.

We have read and understand Mount St. Mary Academy’s Technology Acceptable Use Policy and agree to abide by the terms of the policy.  We understand that the use of computers including internet access is a privilege and not a right.  As a student, I understand that failure to comply with the policy could result in disciplinary action.  As the parent/guardian of this student, I have reviewed this policy with my child and grant permission for her to use her laptop and have Internet access.
