Assumption of Risk: I understand that participation in performing arts, drama games, vocal training, and movement activities involves inherent risks of minor physical injury (such as trips, slips, or falls). I voluntarily assume all risks associated with my child’s participation in these activities at the private studio location.

Medical Fitness & Emergency Treatment: I certify that my child is physically fit and capable of participating in these classes. In the event of a medical emergency, I authorize the staff of Page to Stage Performers to obtain necessary emergency medical treatment for my child if I cannot be reached immediately. Any medical expenses incurred will be my financial responsibility.

Release of Liability: I hereby release, waive, discharge, and hold harmless Page to Stage Performers, its director (Barbara Heller), instructors, staff, and the owners of the private dance studio premises from any and all liability, claims, actions, or demands arising out of any injury, loss, or damage sustained by my child during or as a result of the program.

Property Loss: I acknowledge that Page to Stage Performers is not responsible for any personal items that are lost, damaged, or stolen during class hours.
