Registration Form Information
If your child is a Primary School student in 2024, please complete this form to register for Student Care Services.

Please note that all registrations are subject to space availability and completion of this form does not guarantee a space. We will contact all applicants to inform you of the outcome of your registration.

This form may take up to 15 minutes to complete.

Your email address will be recorded as part of this application. However, Raffles Student Care shall use the email address(es) of the primary Caregiver listed by you in Section 7 of this Form for all correspondence.


Please have the following information and documents at hand when completing this Registration Form:

1. Student's personal details (e.g. name, birth date)

2. Student's medical history records

3. All caregivers' details* (e.g. Name, NRIC, phone number)

^ A clear image of the document taken by a phone camera is acceptable. If you are using a device with a camera (e.g. handphone or tablet), you may select "Upload > Select files from your device" to take a photo of the document when filling in the form.

* Caregivers are adults authorised to bring the student home from the Student Care Centre (SCC).

Personal Data Protection Act (PDPA)
The Personal Data Protection Commission of Singapore (PDPC) has issued Advisory Guidelines on the collection and use of the National Registration Identity Card (NRIC) by business organisations which will come into force on 1 September 2019. As such, this Registration Form do not require the upload of soft-copies of the Student's Birth Certificate (BC), and the parents' NRIC at this point of form filling.

However, Raffles Student Care may, if the need arises, request to verify the identities of the parent(s) and student during a later stage in the registration process.

We will only retain the data of successful applicants and delete the data of all unsuccessful applicants.
Student Care Financial Assistance (SCFA) for lower-income families
We are able to assist students eligible for MSF's Student Care Financial Assistance (SCFA) scheme (i.e. Household Income $4,500 or less, or Per Capita Income $1,125 or less) submit their application requests to MSF.

Parents, whose children are eligible for SCFA, shall submit a Letter of Undertaking before Raffles Student Care offers, on a goodwill basis, partial payment of school fees while waiting for SCFA approvals. Please approach our staff for more information.

PDPA for SCFA Applications
To assist in the SCFA application, Raffles Student Care requires a hard-copy of the parents' NRIC and the child's BC, along with income statements and any other necessary personal documentation. These documents are solely for SCFA applications to the Ministry of Social & Family Development (MSF) and shall not be for any other purposes.

Raffles Student Care shall dispose of all existing hard-copies of the Students' BC and parents' NRIC from our locked cabinets by shredding these documents within 3 years upon approval of the SCFA for the student, as stipulated by current MSF data storage requirements.

For all SCFA re-applications (every two years or less), Raffles Student Care shall ask for a fresh copy of all necessary documents, including the parents' NRIC and child's BC, for the re-application process.

Admission for School-based SCCs
School-based SCCs DO NOT practice the first-come-first-serve policy.

For the mass Primary 1 registrations for 2024 enrolment, we shall collate all registration forms only after the cut-off dates for processing.

For our school-based SCCs, registration forms will be prioritised based on:
  1) Students who are Singaporeans / PRs

  2) Students who are eligible for MSF's Student Care Financial Assistance (SCFA) scheme,
       (Household Income $4,500 or less, or Per Capita Income $1,125 or less)

  3) Students with parents working full-time jobs.

We will process the Registration forms only after the submission deadline. These forms will then be submitted to the Primary School's management for any prioritisation requirements and admission approvals.

Admission for Community-based SCC
For our community-based SCCs, a first-come-first-served basis is practised. We shall use the official date/time stamp as recorded by this Google Form.

To complete the registration at our community-based SCCs, please

1) fill in this Registration Form,
2) pay for the Registration Fees, and
3) pay a one-month deposit.

* Please note that all registration fees and deposits are non-transferable and non-refundable.
Admission for KCare Services
Please approach the MOE Kindergarten to register for our KCare services.

Our KCare Supervisors shall liaise directly with the MOE Kindergarten.
