Mission Monday

On Monday January 18th, First Kids at FUMC Waxahachie will be hosting children to complete a mission project at Prince Hall Fraternal Cemetery in the morning, and then after lunch we will visit the Ellis County African American Museum to watch a film about MLK Jr. and do a Q&A with Dr. Jamal Rasheed. Mission Monday is from 9am-3pm. All school age children are invited, masks are required. Please send a sack lunch, an afternoon snack will be provided. Due to Covid-19 we are limiting registration to 15 children, so sign up early!

Permissions:

Authorization given for medical treatment as defined: In the event that he/she is injured while participating in this activity and requires the attention of a doctor, we consent to any reasonable medical treatment as deemed necessary by a licensed physician. In the event treatment is called for which a physician and/or hospital personnel refuse to administer without our consent, we hereby authorize any adult sponsor representing FUMC Waxahachie to give such consent for us if we cannot be reached by telephone at one of the numbers indicated on this registration, or because of an emergency there is no time for us to give consent. We agree to hold such person free and harmless of any claims, demands or suits for damages arising from the giving of such consent as long as the treatment is administered by or under the supervision of a licensed physician.

Publicity release: I grant permission to FUMC Waxahachie for photographs and video footage of my child to be included in publicity info such as news releases, videos, newsletters, brochures, social media, and web site postings. I understand that the pictures and video footage will not include the names of the students.

I agree to hold FUMC Waxahachie and all volunteers and staff free from liability from all injuries, damages, or losses unless caused by the willful or intentional conduct on the part of the leader or staff.
