Community Collection Nomination Form

Community Collections are personal playlists of favorite learning resources (classes, TED
Talks, podcasts, books, etc.) on a particular subject that are put together by South Bend residents. There are Community Collections on buying a first house, fixing your bike, getting started with computer programming and much, much more.

This form will allow you to nominate your neighbors, friends and coworkers to be the "DJ" for their very own Community Collection. Please complete the questions below to nominate someone you know to curate a collection (you may nominate yourself).

After completing the form, a member of the library staff will follow up with you if your nomination is chosen to become a Community Collection.
