2019-20 DCSD School Supply Request for Students
TO REQUEST SUPPLIES FOR YOUR STUDENT(S) FILL OUT THE FORM BELOW. Our school supply packs are based on Douglas County School District's suggestion for basic supplies at each grade level and may not match the specific request of individual classroom teachers. We will do our best to fill every request, but are limited to the supplies donated. This form may be submitted on-line, or downloaded and returned in person or by mail to: The Foundation for Douglas County Schools, 620 Wilcox St., Castle Rock, CO 80104.
