We (parent/guardian and student) have read and reviewed the above STAMP/SPACE College Preparatory Academies requirements. We understand all requirements stated and agree that my son/daughter will follow all STAMP/SPACE requirements and in the BISD Student/Parent STAMP/SPACE Handbook for the duration of his/her enrollment in the STAMP & SPACE College Preparatory Academies. By submitting this google form, we (parent/guardian and student) acknowledge that we are informed about the requirements of this program and where the handbook can be found. We (parent/guardian and student) understand the students in this program will be held accountable for their behavior and requirements as outlined in this document as long as the student remains in the STAMP & SPACE College Preparatory Academies.

The Curriculum for these academies is an advanced curriculum so all students must meet the prerequisites for the Honors, Advanced Placement (AP), or College Credit (DE) classes. All STAMP/SPACE students are expected to achieve and maintain exemplary academic status.

I, Student, agree to meet the following standards listed below for the duration of my enrollment in the STAMP & SPACE College Preparatory Academies. I understand that failure to meet any of these program standards or other criteria deemed necessary for my success is grounds for review and possible dismissal. These standards include but are not restricted to the following: *

I will adhere to maintaining a demeanor and appearance that is of a professional standard on and off campus due to the nature of working with professionals in the respective fields of study.

I will demonstrate mature behavior as it pertains to a professional setting

I will abide by all of the program requirements located in the STAMP/SPACE Student/Parent Handbook. www.bisd.us/stampspace

I will follow all rules listed in the BISD Student/Parent Code of Conduct Handbook.

I understand that if I am sent to an alternative campus setting due to discipline issues, I will be removed from the academies.

I understand the STAMP & SPACE curriculum (9th-12th grade) consists of all advanced classes, therefore, all core area classes must be taken at the Honors/AP and/or Dual level.

I will meet all prerequisites as required for the Honors/AP/Dual programs.

I will be committed to completing and turning in my Fall Semester project at each grade level.

I understand that as long as I am in the STAMP & SPACE Academies, if I fail a class at the end of the Fall semester, I will be placed on academic probation.

I understand that as long as I am in the STAMP & SPACE Academies, if I fail a class or fail to submit Fall Semester project at the end of the year, I will be removed from the academies.
