I understand that once I join as a member of the DISCOA Junior Board, I am responsible for a $10.00 annual membership fee (dues) each year that I serve, payable by July 1st.
I understand the expectations of a Junior Board member to include:

Attend and participate in meetings as often as possible

Attend and participate in Junior Board functions and events as often as possible

Bring my ideas to the Junior Board

Participate in committees, as needed

Be a team player and have fun!
DISCOA members and Junior Board members may, from time-to-time, be photographed and/or recorded on video. These photos/videos may be shared on social media or the Board’s website. By agreeing to be a DISCOA member or serving on the Junior Board, I give my consent that I (or my minor child) may be photographed or recorded, and that his or her image may be used in future DISCOA and/or DISCOA Junior Board publications and/or social media club advertisements.
Joining the DISCOA Junior Board serves as parental permission for your child (or you) to attend periodic events either at the rink or off-site and that you do not hold DISCOA or DISCOA Junior Board liable for any accidents or mishaps.
My signature indicates my understanding of the expectations of a DISCOA Junior Board member and my willingness to abide by these expectations. I also provide my approval to be contacted (or my minor child to be contacted) by the Junior Board via phone, text and/or email messages.
