Spring Break Registration

Pricing is $70 for the day or $270 for the week.    
Registration fee is non-refundable. $100 each Child 
*Registration fee is separate from weekly tuition
Programming hours are 6:30am-6:00pm. We would like all participants in attendance by 9am.  
Participants will need to bring a NUT FREE lunch daily.
We provide an AM/PM Snack.
Please make sure your child brings a water bottle, athletic shoes, gym-appropriate clothing, and a backpack
Same day registration may not be available.

Once this form is filled out your if you are already on our Brightwheel app please verify all paperwork is completed. 
If you are new to our program, please be on the lookout for an email to get you set up on our Brightwheel app.

How did you hear about us? *
I understand that if I need any changes to my child's registration days, I need to notify the program director by email: afterschoolsports@milwaukeeyard.com to make those changes, and that these changes must be made 2 weeks in advance of the scheduled Day Out. If I change or cancel after the 2 week period the day or days signed up for are still billed.

I understand that Spring Break field trips are subject to change based on facility availability and weather. In the event of cancellation, an alternate field trip or programming at the Milwaukee Yard will take the place of the field trip.
