Form is to be completed by the primary member wanting to make changes to their Adrenaline Fitness membership. Please double check contact information and allow 3-5 business days for processing. A 30 day notice prior to your next billing date is required for all membership cancellations. Memberships on "freeze" must be re-activated prior to cancelling.
