All responses on this form are time-stamped to facilitate fair placement on a first come, first serve basis. This is an elective clinical rotation offered to 4th year veterinary students. This is NOT the Intro to Shelter Medicine course offered to 2nd and 3rd year veterinary students.

(Please email a copy of your vaccination record to ocsvolunteer@gmail.com or bring it to clinic if you have not already done so. You cannot be assigned to a station that requires a rabies vaccination until the record is received.)

Are you interested in being a station captain for this clinic? *
Station captains make sure volunteers have all the supplies needed, answers questions about stations in their area (or get a coordinator if they do not know the answer), and are in charge of making sure their station area is clean afterwards.
What is your availability on the day of the clinic? *
(Animal contact/clinical positions are typically available from 7:00am-3:00pm. If you sign up for an a.m. shift and MUST leave by early, please let us know ahead of time so we may plan accordingly. The time the station wraps up is dependent upon on the # of cats checked in, # of surgeons and flow of surgery).
Are you available to help with clinic set-up on Friday, January 24th, 2020 starting at 5:30 p.m.? *
We highly encourage first time volunteers to attend set-up! It is a great opportunity to learn about the clinics before the actual day! We can give you a tour of the surgery room and answer any questions you might have!
