Please complete this Transportation Registration Form if you live within the Chariho Regional School District and your child will require bus transportation to or from your home to one of the Chariho schools or private schools in the district.  If you do not require bus transportation for your child, you do not need to complete this form.  
NOTE:  You can not change your address with this form, the bus will pick up the student with the address we have on file.
