Please complete the following application to be considered for a 2026 Hazelwood PTA Council Scholarship.

Questions with a red asterisk are required.  We recommend you download the application questions at this link and prepare your answers offline before filling out this form online.

We recommend you use your personal email account to submit this form.  Using a Gmail account would be best, and it will allow editing privileges.   You may return to this form and edit your responses before the deadline by following the link that will be emailed to you after you submit your responses for the first time.  The application will be closed on ***Tuesday, March 31st, 2026 at 11:59 pm*** and no additional applications or further changes will be accepted after that date.

Each year the HSD PTA Council has provided $500 scholarships for up to 100 graduates from our three high schools and the Opportunity Center.

Any questions regarding the scholarship should be sent to hsdptascholarshipinfo@gmail.com. PLEASE DO NOT CONTACT THE HSD ADMINISTRATION. Application must be electronically submitted no later than ***Tuesday, March 31st, 2026 at 11:59pm.*** Submissions after this date and time WILL NOT be accepted.

INSTRUCTIONS:
All correspondence regarding the scholarship will be done electronically. IT IS VITALLY IMPORTANT THAT YOU PROVIDE VALID CONTACT INFORMATION, INCLUDING AN EMAIL ADDRESS THAT YOU CHECK REGULARLY.  DO NOT wait until the last minute to complete the application. Some of the information can only be provided by your counselor. It is recommended that you print a copy of the application to complete first, then transfer your responses to the electronic form. Please note that incomplete applications WILL NOT be accepted.  PLEASE CHECK YOUR ELIGIBILITY BEFORE COMPLETING APPLICATION (see requirements here).  Only eligible applications will be considered.

SCORING:
Applications will be scored based on content. Please pay close attention to the short essay questions. Your answers should reflect a carefully thought out response. Proper grammar and spelling are equally important. Be sure to have someone proofread your responses.

AWARD NOTIFICATION:
Scholarship recipients will be notified of their award by email.  Please be sure to provide an email address that you check regularly.  Winners will receive an award letter and acknowledgement form that will need to be returned along with requested documentation BEFORE scholarship check can be released.  Additionally, scholarship may be forfeited should recipients fail to respond and submit required correspondence before the last day of school.
