CTUIR Title VI Youth Services Program Needs Assessment Survey

The Youth Services Program is a unique opportunity to provide our students, community members, and school staff with specific resources and experiences that in-turn create a safe, culturally appropriate environment where identity and confidence can be fostered and embraced within our students. It is important to us that we provide our families a platform to voice their wants and needs for their students. We plan on using the information gathered in this needs assessment survey to help determine the future services, activities, and offerings from the CTUIR Title VI Youth Services Program for our students and community.

Please note: The raffle associated with this survey is now closed. However, please proceed to provide your feedback!
