KISD Instrument Usage Fee Agreement (Cello and Bass Students Only)
A yearly maintenance fee of $80 will be collected from each student who uses a Katy ISD owned instrument for Orchestra. Once issued, instruments become the sole responsibility of the parent/guardian of the student. In the event of loss or damage (to include stolen instruments) caused through carelessness/negligence of the student, the parent/guardian is responsible for the repair or replacement of the instrument.
Katy ISD will provide adjustment and repair to the instrument without any charge as long as the repairs are not a result of the negligence or willful destruction. Free service includes regularly scheduled maintenance of the instrument.
Return and Refund Policy:
Instruments are issued for the academic school year. Instruments must be returned before the last day of school, unless written permission to have the instrument over the summer is received from Mr. Katz.
If any damage is noted upon receipt of instrument, or if repairs are needed, contact Mr. Katz immediately.
This year there is a waiver form application that needs to be approved by Principal Hinson. If you are unable to pay please have this waiver form printed and turned in to Mr. Katz no later than Friday, 9/11. There will be hard copies of this form at school as well.
Payments are only able to be made through RevTrack in the Pay N' Go System.
