Rules & Guidelines:
  1. Your request must be approved before the date & time of your requested lab section
  2. Only 2 lab reschedules are allowed per semester

Please make sure to DOUBLE CHECK all of your entries before submitting as repeated forms submitted for the same student will not be approved once the cap of 2 has been reached.
