Please complete this form if your student needs childcare and will be picked up and/or dropped off by a school bus at an address other than the home address. Childcare is considered as daycare or situations where custody is shared or split between parents/guardians. Daycare includes students utilizing a private or commercial daycare. Students will not be assigned to more than two buses, i.e. 1) bus based on home address 2) bus based on childcare. Additional bus assignment requests are not guaranteed during the first 10 days of school. After the first 10 days of school, requests may take 3-5 days to process and become effective. **Requests may not be made for reasons other than childcare.
 
ALL LOCATIONS MUST BE WITHIN THE SCHOOL'S ATTENDANCE AREA FOR BUS TRANSPORTATION.
