Device Requests / Swaps
Please use this form to indicate new device requests or device repair needs. Be as specific as possible when gathering request information. This will help in the determination of whether a new device is needed or repair possible on site.
**HOTSPOT REQUEST?** All hotspot requests must be vetted by the district. By completing this form, you ensure that a request will be placed for that family according to district protocol. Determinations typically take 2-5 days before devices are approved and delivered to the school.
**HOTSPOT ISSUE?** Families should call T-Mobile directly using the phone number on the device. T-Mobile is providing technical support for their devices.
Please DO NOT guarantee families same day service with devices. Depending on stock and any special circumstances or conditions, it could take a week or more to make sure this request is ready for pickup.
