**HOTSPOT ISSUE?** Families should call T-Mobile directly using the phone number on their existing device. T-Mobile is providing technical support for their devices. All NEW hotspot requests must be submitted to District ticket after your response to this form is processed. This does take time to process.

We DO NOT guarantee families same day service with devices. Depending on stock and any special circumstances or conditions, it could take a couple days to make sure this request is ready for pickup.
