Boat License Information
This form must be submitted once for each boat to be licensed. Please provide ALL of the information for the FIRST boat, or at least all you know.

For boats after the first, select "Submit an Additional Form" at the end until you are done. On subsequent forms you may skip certain fields (as noted) if they are the same as a boat you have previously registered.
[Rev. 10DecA]

Property Owner's Name or ID *
Every property owner has an ID, but most don’t know what it is. We can look it up, but only if the name (or some other piece of info) we are given is an exact match to our records.
Your answer
Email
REQUIRED for 1st Boat only -Your preferred email address. Helpful for e-processing and e-Pay. (can type ‘SAME’ for boats after the 1st)
Your answer
Phone number
REQUIRED for first boat only - Where we can reach you or at least leave you a message. (can type ‘SAME’ for boats after the 1st)
Your answer
Lot Number *
REQUIRED for first boat only. If not a property owner or resident, type "NR". Can enter "SAME" if same as a previous boat registered. Can also type ‘SAME as 2017’ if that is the case. Can be either Lot Number (i.e., Fairway 67) or the Property Address to which each boat should be assigned. If not specified we will apply boats to your properties based on our best guess. For boats previously licensed, we can look up the property ID from last year. This is important because property owners get very deep discounts (up to $1,425) on their first (Primary) and second (Secondary) licenses, and for each lot owned. If we cannot readily verify property ownership we cannot grant the discounts, or maybe any license for that matter.

Boat Make and Type *
If available, this would be the make or Manufacturer as shown on the state registration, such as Aqua Patio, or Godfrey or Tige or Malibu. If the craft is not registered, the type of craft alone will suffice, such as canoe, kayak, sailboat, paddle-boat, paddle-board. - This information is typically on the state registration certificate and does not change from year to year. If you are certain your boat was registered by you last year, you may simply record ‘SAME’ on the form. You must, however, give us sufficient information to recall last year’s data, or you may attach or e-mail a copy or photograph of the state registration, past or current.
Your answer
Boat Length *
Decimal feet or feet and inches. By submitting this form you are certifying this complies with our Boating Ordinance(s). This may be subject to verification for new boats per Ordinance. This information is typically on the state registration certificate and/or title or placard, and does not change from year to year. If you are certain your boat was registered here at the Lake by you last year, you may simply record ‘SAME’ on the form; you must, however, give us sufficient information to recall last year’s data. If it is close to the limit, or questionable, you may send or e-mail a copy or photograph of the title, placard or state registration, past or current. We will accept state-issued documentation. If in doubt, we can have the police measure it...
Your answer
Hull Number
If available. This is typically shown on the state registration certificate, or your title, or your insurance policy. We likely would only need this to track down ‘found’ craft floating around the Lake.
Your answer
State Registration Number
– Such as PA-1234-AB. This info, for all powered boats, is essential. Boats without state registration will not be issued any Powered license. Leave blank or state ‘N/A’ on the form if not registered and non-powered.
Your answer
One or Two Year Expiration *
Please make sure you submit payment for two years if you choose that option. Since we will be going to ONLY a 2-year registration period in the future we are offering the one-year option to match up years and colors with your State stickers. (this year only...)
Insurance Company Name *
May be SAME if same as previous boat - The name of the carrier issuing coverage/policy, such as Allstate or State Farm or Erie. - If this remains the same as last year, you may simply record ‘SAME as 2017’ on the form.
Your answer
Insurance Policy Number *
May be SAME if same as previous boat - The unique policy identifier for your liability coverage on each craft. - If this remains the same as last year, you may simply record ‘SAME as 2017’ on the form.
Your answer
Insurance Policy Expiration Date *
MM/DD/YYYY please. May be SAME if same as previous boat. Most policies will expire before the end of the registration period. It is your responsibility to maintain a valid policy. The Borough assumes no liability in any event, but this provides some level of diligence and assurance for the protection of all boaters, including the owner.
